Family sources stated that the monarch was receiving treatment for cancer, following weeks of public concern regarding his health condition.

Accession at age three and reign
Oyo ascended the throne on 12 September 1995 at three years of age following the death of his father, King Rukirabasaija Patrick David Matthew Kaboyo Olimi III, who died of a heart attack. He assumed full royal authority in 2010 upon celebrating his eighteenth birthday. In 2025, Guinness World Records officially recognized him as the youngest reigning monarch in the world.

Uganda operates as a constitutional republic under President Yoweri Museveni, but the national constitution recognizes five traditional pre-colonial kingdoms. Traditional rulers are legally barred from participating in partisan politics, serving instead as cultural custodians and ceremonial leaders.
His Majesty leaves behind a Prince as his heir, whose identity will be formally revealed at the appropriate time, in accordance with the traditions and customs of Tooro Kingdom.
