Mother of three dies after trench collapses during septic tank works in County Limerick

A workplace incident in County Limerick on Friday resulted in the death of a mother of three after an excavated trench collapsed during site works. The victim has been identified locally as Elizabeth Lynch, also known as Liz, a resident of the Newcastle West area who was in her 40s. The fatal accident occurred at a work site in the village of Kilmeedy, situated a short distance outside Newcastle West town.
At the time of the collapse, excavation work was actively taking place at the premises. Lynch was involved in digging an earthen trench designed for the preparation of a septic tank and its accompanying pipe infrastructure. The trench suffered a sudden structural collapse, falling inward directly on top of her while the ground preparation work was underway.
Emergency response and death confirmation
Immediately following the trench collapse, other individuals present at the Kilmeedy premises rushed to intervene and provide assistance. Despite the rapid rescue and first aid efforts attempted by others at the scene, the injuries sustained during the cave-in proved fatal. Lynch could not be resuscitated by those present at the property.
An Garda Síochána were alerted and dispatched officers to the property in Kilmeedy on Friday. Upon arrival at the premises, emergency and police personnel assessed the scene of the incident. Lynch was officially pronounced dead at the scene by authorities, who secured the excavation area.

Garda referral and HSA statutory inquiry
Following the formal confirmation of the fatality at the site, An Garda Síochána initiated preliminary reviews into the events surrounding the workplace accident. A garda spokesperson confirmed that officers attended the scene of a fatal workplace incident involving a woman aged in her 40s in Kilmeedy on Friday.
Because the fatality occurred during construction and ground excavation duties, gardaí referred the case to Ireland's statutory workplace safety regulator, the Health and Safety Authority (HSA). The HSA confirmed that its inspectors had been notified of the fatal workplace event and commenced a formal investigation into the physical site conditions and the circumstances of the trench excavation.
The Health and Safety Authority is aware of the incident and has launched an investigation.

News of Lynch's death prompted an outpouring of grief across the Newcastle West and Kilmeedy localities. Lynch was a prominent figure within local youth sports, dedicating time to coaching young girls and participating as an active member of Monagea Ladies Gaelic Football Club.
Elected representatives and community leaders expressed profound sadness over the loss. Local councillor Adam Teskey described the deep emotional impact felt across the surrounding district following Friday's event.

Councillor Teskey stated that the tragedy has taken a devastating toll on the community, extending condolences to Lynch's family. In response to the tragedy and as a mark of respect to the Lynch family, Monagea Ladies Gaelic Football Club suspended all sporting fixtures and club activities for the weekend. Newcastle West Town FC similarly cancelled and suspended all weekend football matches and related events.
