Rai 1 sets Doc Season 4 premiere for 1 October as cast addresses healthcare cuts

The fourth season of Italian medical drama Doc - Nelle tue mani will premiere on Rai 1 and RaiPlay on Thursday, 1 October. Produced by Lux Vide, a Fremantle company, in collaboration with Rai Fiction, the series will air across eight prime-time evenings with two episodes broadcast each night, totaling 16 fifty-minute episodes. Luca Argentero returns to the role of Dr. Andrea Fanti following an in-story absence of more than a year. Upon his return, Fanti discovers substantial changes at Policlinico Ambrosiano, where Dr. Giulia Giordano, portrayed by Matilde Gioli, has been promoted to head of Internal Medicine.

Andrea Fanti returns after over a year of absence and finds a hospital he no longer recognizes, but above all he is forced to reckon with himself, with the need to put back together what he has lost.

Budget cuts and investigative plotlines

The new episodes center on administrative and financial pressures within the public hospital system. Giordano manages a department facing financial deficits while navigating demands from the new medical director, Stefano Campigli. Campigli implements operational cuts to prevent hospital insolvency and a potential acquisition by a private holding company. These measures reduce the time clinicians spend listening to individual patients in order to increase patient turnover, directly conflicting with Fanti's patient-first philosophy. Fanti initiates a push for more physicians, nurses, and beds, a campaign that creates professional friction with Giordano. In addition, Fanti and an idealistic new resident named Matteo investigate a series of unexplained patient deaths across multiple Ambrosiano wards.

Cast updates and production leadership

Directorial responsibilities for the 16 episodes are divided between Nicola Abbatangelo, who directed episodes 1 through 8, and Alessandro Tonda, who directed episodes 9 through 16. Returning ensemble cast members include Pierpaolo Spollon, Marco Rossetti, Elisa Di Eusanio, Laura Cravedi, Elisa Wong, Sara Lazzaro, and Giovanni Scifoni. The fourth season introduces Cristina Marino, who is married to Argentero, in the role of locum physician Dr. Lidia Morganti. Argentero noted during the press conference that while he initially hesitated when producer Luca Bernabei proposed the casting over potential claims of favoritism, their personal dynamic supported the developing relationship between their characters on screen.

Real-world healthcare discourse and future plans

At the official launch at Rai headquarters on via Alessandro Severo in Rome, the actors framed the plot around the current condition of Italy's National Health Service. Argentero argued that universal healthcare represents an essential right alongside education and public safety, questioning political choices to reduce medical spending. Gioli noted that the series provides a platform to portray the operational strain placed on clinical personnel due to budgetary constraints. Screenwriters Viola Rispoli and Francesco Arlanch confirmed during the launch that writing work is already underway for a potential fifth season.

Argentero remarked that continuing the series remains subject to story quality and audience ratings for the upcoming season, adding that while imagining the show without Fanti would bother him, long-running productions must eventually end.
