
Spain faces Alzheimer's drug funding dispute as global dementia cases rise
Ahead of World Alzheimer's Day on 21 September, Spanish patient groups and neurologists are demanding public coverage for new slowing therapies, while blood biomarker testing gains ground.
Funding dispute over new treatments in Spain
As World Alzheimer's Day approaches on 21 September, Spanish patient groups and neurologists are contesting the government's decision to exclude disease-modifying therapies from public coverage. In July, the Interministerial Commission on Drug Prices (CIPM) rejected public funding for lecanemab and donanemab, two monoclonal antibodies that slow cognitive decline by 30%. While approximately 50 countries have authorized or integrated the treatments, patients in Spain can currently obtain them only through private clinics at an estimated cost of €50,000 to €60,000 for an 18-month cycle, or roughly €25,000 annually before diagnostics. Dr. Juan Fortea, neurology unit director at Hospital de la Santa Creu i Sant Pau in Barcelona, calculates that between 85,000 and 100,000 early-stage patients meet theoretical eligibility, though clinical constraints such as APOE4 gene limits and microbleed thresholds will keep actual recipient numbers much lower.

Rising prevalence and family economic burden
Dementia affects more than 57 million people worldwide, with nearly 10 million new diagnoses recorded each year. According to data from the Spanish Society of Neurology (SEN), more than 800,000 people live with Alzheimer's disease in Spain, where roughly 40,000 new cases are diagnosed annually, over 65% of them in women. In 2025, Alzheimer's and related dementias caused over 36,000 deaths in Spain according to the National Statistics Institute (INE). The Spanish Alzheimer Confederation (CEAFA) estimates the domestic annual cost of the illness at €35 billion, while global costs reach $1.3 trillion. A study by the Pasqual Maragall Foundation found that the average economic burden per patient reaches €42,000 annually, rising above €77,000 in advanced stages. Families cover 86% of these expenses directly, whereas public healthcare spending accounts for 8%.

Advances in blood biomarkers and diagnostics
Diagnostic protocols are changing following research on blood-based biomarkers. A clinical study led by Lund University in partnership with the Pasqual Maragall Foundation demonstrated that measuring the p-tau217 protein in blood plasma achieves 81% precision in detecting amyloid pathology. Implementing a two-step screening model (a plasma test followed by confirmatory imaging only if positive) reduced the requirement for invasive lumbar punctures and tau PET scans by over 40%. At the Alzheimer's Association International Conference (AAIC 2026), updated data indicated that plasma p-tau217 performance is non-inferior to amyloid PET imaging in identifying disease markers among asymptomatic individuals.
Immune system mechanisms in neurodegeneration
Basic scientific research published in Nature Neuroscience points to systemic immune dysfunction alongside classic amyloid and tau accumulations. Laboratory models showed that Alzheimer's disrupts myeloid cell production in bone marrow, where elevated type I interferon signaling impairs the maturation of monocytes that travel to the brain. Researchers are investigating how central nervous system immune cells, including microglia and their surface receptor TREM2, modulate plaque accumulation and inflammatory responses.
Dr. Xavier Montalban, medical director of Ace Alzheimer Center Barcelona, described the role of inflammatory pathways in disease progression.
Alzheimer's occurs not only due to an abnormal deposition of beta-amyloid or tau proteins in the brain, but there is also a component of inflammatory molecules.
