Sequence of events during machine maintenance
Initial findings indicate that the fatal incident took place while the employee was carrying out assigned duties during an early shift on the shop floor. The hydraulic press had developed an operational malfunction, prompting the worker to attempt a repair on the industrial machinery independently. According to details established at the scene, the employee attempted to inspect and clean a faulty sensor attached to the unit. The worker did not shut down or disconnect the electrical power supply from the machine before beginning the maintenance procedure. As he was servicing the equipment, the hydraulic press restarted unexpectedly without warning, trapping and crushing the employee within the operational mechanism.
