Brazilian investigators cite severe icing, de-icing system failures, and crew distraction in the August 2024 Voepass crash that killed all 62 aboard near São Paulo.
The final report
Brazil's Center for Investigation and Prevention of Aeronautical Accidents (Cenipa) released its final report on the 9 August 2024 crash of Voepass Flight 2283 on 23 July 2026. The ATR 72-500 twin-turboprop was flying from Cascavel, Paraná, to São Paulo's Guarulhos international airport when it entered a flat spin and crashed in a residential neighbourhood in Vinhedo, about 80 km northwest of São Paulo. All 62 people on board died, making it the country's deadliest air disaster since 2007. Lieutenant Colonel Paulo Mendes Froes told reporters that 19 factors contributed to the accident. The findings were presented to victims' families before the public release.
Icing and mechanical failures
The aircraft encountered severe atmospheric icing while cruising at 17,000 feet (approximately 5,200 metres). The de-icing system experienced multiple failures and was switched on and off several times by the crew during critical phases of the flight. Cenipa found that the ice buildup caused a loss of lift, sending the plane into an unrecoverable flat spin that was captured on video by residents. The aircraft lost nearly 5,000 metres of altitude in roughly one minute. A preliminary report in 2024 had already identified signs of ice accumulation but did not determine a definitive cause.
Crew distraction and missed warnings
Flight recorders showed the pilots were distracted by routine radio communications and administrative tasks, and engaged in informal conversations unrelated to flying. Eight "cruise speed low" warnings sounded during the flight, but the crew did not carry out the required procedures. The report described an "inattentional blindness" that prevented timely corrective action. The only cockpit mention of ice accumulation came two minutes before impact, and no emergency was declared. The crew also did not modify the flight plan to avoid cloud formations along the route.

Voepass safety culture
Investigators identified a systemic "culture of normalizing deviations" at Voepass. Known mechanical issues, including the de-icing fault, were tolerated before takeoff and not recorded in the aircraft's flight logs. The airline had a recurring practice of not reporting faults or reporting them informally. Voepass filed for bankruptcy protection in April 2025, a month after Brazil's aviation regulator ANAC suspended its operations citing safety concerns. The report noted that company officials were aware of the de-icing system problem before the flight but did not document it.
A culture of normalizing deviations took place within Voepass.
Aftermath and recommendations
Cenipa issued a recommendation to the European Union Aviation Safety Agency (EASA), urging it to work with ATR to review the "degraded performance" procedure to help prevent crews from reaching similar limit conditions. ATR, a joint venture between Airbus and Leonardo, did not immediately respond to requests for comment. The crash was Brazil's deadliest since 2007. The report's findings aim to prevent similar icing-related loss-of-control events in the future.
